Do you include these extensions (AS Slider Nivo, AS Slider Moo, as the accordion menu, Phoca Gallery, Google maps plugin, Acymailing Component) in the premium version of the template (002 044), or they require additional payment?

Yes, all these extensions are included, there are no any additional payments.
I purchased this template and installed with quick start. but it works just the home page. As I click on other pages I get this error "404 - Category not found".

Hello MARTUCH,
It looks like your server doesn't support Search Engine Friendly URLs,
please turn off it in the Joomla Admin Panel -> Site -> Global Configuration -> SEO Settings -> Search Engine Friendly URLs.
But this is not just the problem with Galery. I have this same problem with articles. http://pvcsyrena.com/index.php/artykuly-przeglad/84-news-y/107-pvc-syrena-0-3-queens. For 100% that is problem with template. Also you can go here: http://pvcsyrena.com/index.php/artykuly-przeglad and press link to the article. You will see this same problem.
